In order to issue a passport to the citizen of the Republic of Serbia an individual needs to present the following: · Two fully completed and signed passport issuance forms · Three identical photographs, size 3.5 x 4.5 cm (front face, without cap or scarf) · Original proof of citizenship from the Republic of Serbia, not older than 6 months and original Certificate of Birth · Old passport (in order to cancel it; it is then returned to the client) · Proof of the regulated stay in Canada. Note: In case of the loss or damage to the passport it is necessary to provide: a) police report about the loss/disappearance of passport b) filled out form about the loss/disappearance of passport c) written statement about the disappearance/damage of the passport d) proof of identity (old passport, personal ID of drivers license from the Republic of Serbia) For issuance of child's passport (request signed by parents) it is necessary to provide following documents: a) original proof of citizenship of the Republic of Serbia, not older then 6 months from the date of issuance (and for replacement of the blue passport) or proof that the child was registered through a diplomatic-consular mission of the Republic of Serbia b) photo-copy of parent’s passports Entry into passport of parent is valid for two years only and can be done only once, into passport of only one parents, If a child was born in Canada and its passport is being issued for the first time. If passport is being changed because of the change of last name: a) due to the marriage or b) due to the divorce. If the request is being sent by mail, it is necessary to enclose a self-addressed, prepaid return envelope.
TO ISSUE THE PASSPORT, THE AGREEMENT OF THE MINISTRY OF INTERIOR OF THE REPUBLIC OF SERBIA IS REQUIRED
Consular taxes for the issuance of passport are payable in cash, by money-order or a Certified Check which must be issued to the Embassy of the Republic of Serbia. |
